Lineup cards are required for all games 7U and up. Coaches MUST bring filled out lineup cards to every game.

Lineup Card Options

1. Get hard copy Game Cards from the Ref Shack at AVS or Parras. 

2. This Excel file prints out names which will match up with the lines on the Game Cards: Game Card Labels
 You can print these out onto Avery labels, or just print onto paper and attach it to your Game Card.

3. This PDF file is downloadable and fillable Game Card. Simply fill-in your team and game information, print, and off you go! Fillable Game Card


All of the information below should be filled out on your Game Card BEFORE the game.
